Please remember all Student-Athletes must have the following forms fully completed and turned into the Athletic Department before they can participate in any athletic activity.

Please also understand that even if the information was accurate at the time that you enrolled or re-enrolled your child(ren), the information may have changed prior to commencement of classes or during attendance at the school. Please also note that only the Head of the School in which your child(ren) are enrolling have the authority to make commitments regarding the nature of the program, specific arrangements for your child(ren), or other changes from the school's regular curriculum.
